1) The SUBSURF post-processing menu had been broken in
Revision 322. It was giving zero subsurface stress for
all surface pairs other than the first surface pair, ever since
Revision 322.
2) Using a stepped segment in a pinion shaft was causing a failure
during the generation phase.
